Pro Tools Bug - Consolidate Audio Clips Causes 1000Hz Sine Wave - Anyone Else Experience This?

Over the last several months I have been experiencing a very odd Pro Tools Bug. When I use Consolidate Clip to consolidate audio clips in Pro Tools I occasionally end up with a 1000Hz sine wave. This forces me to Undo the Consolidate Clip action and try again. However, this can cause the same problem. As far as I can tell this is a random Pro Tools bug that occurs in both heavy track & plug-in sessions and naked basic two audio track sessions.

This Pro Tools Bug isn't exclusive to consolidating multiple audio tracks at a time, the sine wave can be produced when working on single audio tracks as well.

Has anyone else in the Pro Tools Expert Community experienced this bug?

I am running Pro Tools HD 12.7 on OSX 10.11.6
